Use of English Part 4: Mixed tenses

Part 4 of the Use of English part has a section in which you are to decide the right tense to fill a gap (and write it correctly). The verb is given. This is the example provided at the official sample on http://www.caib.es/govern/sac/fitxa.do?estua=924793&lang=ca&codi=1002198&coduo=924793:

 At the Dentist’s 

I was on time for my dentist's appointment, but the dentist was still busy with another patient, so I (0) ...sat... (SIT) in the waiting room and started to read some of the old magazines lying there. While I (1) ... (WONDER) whether to leave and come back another day, I (2) ... (NOTICE) a magazine article about teeth. It (3) ... (BEGIN): 'How long is it since you last (4) ... (GO) to the dentist?. (5) ... (YOU GO) regularly every six months?”.
